SABRE Update

Support for Online Grading
Drop-in/test-drive sessions to help the campus prepare for online grading will be held from 10:00 a.m. to 4:00 p.m. Wednesday, May 5, and Friday, May 7, in E. H. Butler Library 318 (CyberQuad).

Technical support will be available via e-mail at sabresupport@buffalostate.edu and via telephone at ext. 3434 from 8:00 a.m. to 8:00 p.m. during CEP week (May 10–May 14). Support will also be available in E. H. Butler Library 318 (CyberQuad) from 9:00 a.m. to 5:00 p.m. Monday, May 10; Tuesday, May 11; and Friday, May 14.

Technical support will be available in CyberQuad and via e-mail and phone from 9:00 a.m. to 4:00 p.m. Monday, May 17. Any grade submitted after 4:00 p.m. May 17 must be submitted using the "change of grade" form.

Community Service Faculty Fellowships Awarded
The Volunteer and Service Learning Center is pleased to announce that the following Buffalo State faculty members will receive Community Service Faculty Fellowships for summer 2004:

Fellowships were granted to faculty members interested in acquiring an in-depth understanding of service learning pedagogy and in integrating community service into a current or new course. Training will be provided in May and June by national service learning experts and experienced Buffalo State colleagues. Community Service Faculty Fellows receive a $1,500 stipend for participation. StudyQuad Ribbon-Cutting Ceremony
Maryruth Glogowski, director of E. H. Butler Library and Instructional Technology, invites the campus community to attend a ribbon-cutting ceremony and reception for StudyQuad, Butler Library's new state-of-the-art study facility, at 12:15 p.m. (Bengal Pause) Thursday, May 6. President Muriel A. Howard, Provost Dennis K. Ponton, officers of United Students Government, and Glogowski will officiate. Refreshments will be served in the lobby.

Located on the first floor near the library entrance, StudyQuad will provide a safe, well-lighted place for Buffalo State students, faculty, and staff to work late at night. It is wheelchair accessible and will be equipped with 50 new computers, both PC and Macintosh, a wireless area, a scanner, a black-and-white printer, a color printer, a photocopier, study tables, and comfortable seating. Two group study rooms also will be available.

When the library is closed, StudyQuad will be accessible through a separate entrance on the side of the building. For safety and security purposes, Buffalo State students, faculty, and staff members must present valid Buffalo State ID cards for admittance after 11:00 p.m. University Police student assistants (UPSAs) will staff StudyQuad during late-night hours, and the Escort Van will stop every half hour to take occupants to their cars or residence halls.

SkillSoft E-Learning
Employees also may participate in SkillSoft, an online learning program that offers more than 800 courses and a variety of certificate programs that allow participants to demonstrate mastery of a set of skills.

More information about the SkillSoft E-Learning Program is available from the Human Resource Management Web site, www.buffalostate.edu/offices/hr. Employees also may register for courses on the site.

This month's SkillSoft courses may be taken in CyberQuad's Faculty/Staff Workroom, E. H. Butler Library 310, by appointment only; call Paul Reynolds at ext. 3361. Please check in at E. H. Butler Library 315 (CyberQuad) before starting.

Webmaster Training and Web Templates
Web templates are now available for office and department use. The FAST Development Center and the College Relations Office offer a five-course sequential training program especially for Web template users. The course lineup consists of Writing for the Web, Using the Web Template System, Introduction to HTML, Getting Found in Search Engines, and Understanding Site Traffic Logs. Faculty and staff are welcome to take any or all of the five courses offered. Employees may review course descriptions and register at http://bscintra.buffalostate.edu/registration.

Employee Assistance Program
The Buffalo State College Employee Assistance Program (EAP) is an information, assessment, and referral service that provides confidential help to employees with personal problems that negatively affect their lives and job performance.

The program's trained, on-site coordinators provide confidential information and referrals to any employee or supervisor seeking help with a personal or job-related problem. Coordinators are trained to refer employees to the most appropriate community service providers for problems such as emotional stress, depression, financial or legal difficulties, alcoholism, drug abuse, or marital or domestic discord. Coordinators also provide information on local resources for child care, elder care, legal, and financial services.

EAP services are available to state employees, their family members, and state retirees. If you need help with a personal or family problem, please contact one of the following campus coordinators. All calls and discussions are strictly confidential.

Update on the Dean of Students
Phillip Santa Maria, associate vice president for student affairs and dean of students, is at home, recovering from surgery on March 16, and is undergoing radiation and chemotherapy treatment. He has asked that we inform the campus community of his deep appreciation for the many flowers, fruit baskets, books, cards, and expressions of concern he has received. He welcomes hearing from well-wishers and asks that any visitors call ahead.

Confidentiality of Health Information: Policy and Procedure
Statement of Policy
Buffalo State College is committed to protecting the privacy and confidentiality of the health information of the population it serves. Health information is strictly confidential and should never be disclosed or confirmed to anyone who is not specifically authorized to receive the information under the institution's policies or applicable law.

Failure to adhere to state and federal law, the Health Insurance Portability and Accountability Act (HIPAA), or college policies and procedures regarding the confidentiality of protected information will be considered a breach of confidentiality and will result in the imposition of appropriate sanctions.

Scope
The policy applies to all members of the institution's workforce, whether directly employed by the institution or serving under an alternative arrangement. It shall include, but not be limited to:

  • Employees
  • Volunteers
  • All students participating in a health-related program (including medical interns and residents)
  • Agency and contracted staff (including temporary staff)
  • Consultants
  • Contractors and subcontractors
  • Faculty and credentialed staff

Education and Training
Buffalo State College is responsible for providing job-appropriate training to departments that are directly affected by HIPAA or that are recommended to follow HIPAA guidelines. Such training will address:

  1. The need for confidentiality
  2. Types of information that are considered confidential
  3. Sanctions associated with a breach of confidentiality
  4. The institution's confidentiality agreement and the need for annual signature
Confidentiality Agreement
Each member of a Buffalo State College HIPAA-affected or HIPAA-recommended workforce will be expected to review and sign the Buffalo State College confidentiality agreement. This should occur upon hire/affiliation and on an annual basis thereafter. This signed statement will be maintained in the appropriate employee personnel file.

Suspected Breach
Any and all breaches of confidentiality should be reported to the area/department supervisor. Failure to report a breach will be considered a violation of this policy. The supervisor will report the breach to the campus HIPAA privacy officer for investigation.

Sanctions
Upon a finding of a breach of confidentiality by an employee in a collective bargaining unit, the college shall initiate action pursuant to the applicable collective bargaining agreement to implement an appropriate disciplinary penalty. Such penalty may include, but is not limited to, the following:

  • Letter of reprimand
  • Suspension
  • Fine
  • Loss of accrued leave credits
  • Demotion
  • Termination

For employees not represented by a collective bargaining unit, sanctions may include actions up to and including termination of employment.

Athletics News
Lacrosse
Buffalo State's lacrosse team clinched its first-ever SUNYAC playoff berth on April 24 with a 9-6 victory over Oswego. The Bengals wrapped up the regular season with a 4-3 conference record and are currently 9-3 overall. Buffalo State, the No. 4 seed, will face the top-ranked five-time defending champion Cortland Red Dragons in the semifinals in Cortland on Friday, April 30, at 2:00 p.m.

Softball
Buffalo State's softball team closed out the regular season on a 10-game winning streak and clinched the No. 2 seed in the SUNYAC playoffs after a two-game sweep over Geneseo on April 25. The Bengals finished the SUNYAC regular season with a school record for wins at 16-4. Buffalo State will open the double-elimination tournament at Cortland today at noon against Oneonta, the fifth seed.

Track and Field
Justin Muise led the Buffalo State men's track and field team with a pair of championships in the 400-meter dash and 400-meter hurdles with times of 48.89 and 54.56 respectively. Muise also took second place in the 110-meter hurdles with a time of 15.21. Ken Ciolek also captured a pair of titles, winning the javelin (54.66 meter) and the triple jump (13.20 meter). The Bengals' 4x100-meter relay team of Tim Giagios, Chris Lape, Muise, and Mike Taylor took second with a time of 43.14.

On the women's side, Mercedez Roper crossed first in the 100-meter dash with a time of 12.90. She also earned a championship with the 4x100-meter relay team with Desi Gibson, Keyia Thompkins, and Linda Williams, clocking 49.12.

From the Senior Adviser to the President for Equity and Campus Diversity

Affirmative Action Plan
The 2002–2003 Affirmative Action Plan is available for review at the E. H. Butler Library Circulation/Reserve Desk, the deans' offices, the vice presidents' offices, and the Equity and Campus Diversity Office, Cleveland Hall 415.

The report shows that the college has made progress in recruiting, hiring, retaining, and advancing women, members of underrepresented minority groups, veterans, and individuals with disabilities. Continued effort needs to be made in hiring males to clerical/secretarial positions and females for the skilled crafts. The Equity and Campus Diversity Office will respond to questions about the report during regular business hours.

Paper ballots will be tallied after electronic ballots; thus, if a member should accidentally vote twice, only the electronic vote will be recorded.
